The cost of the replacement is $25 and will be emailed (PDF) to you within 24-hrs of your request.  Please provide a correct spelling of your name, mailing address, phone number, and email for us to communicate with you. 

You must be currently certified through Urban Rescue Solutions CPR and AED to be issued a replacement card.
